Hello, I am Dr. Jessica. Welcome to my "Hypertension Medical Course".
In this course, I want to help you thoroughly understand how blood
pressure evolved from a vital sign of life into the number one threat
to human health.
Perhaps this opening statement surprises you. Don’t worry, let’s first
look at what blood pressure is.
Blood pressure is one of the four basic indicators of life, alongside body temperature, heart rate, and respiration. The latter three are superficial, while blood pressure is intrinsic and invisible in life. Throughout evolution, blood pressure has played a crucial role. It meets the ever-increasing energy demands and enables rapid, precise energy distribution. Without blood pressure, we couldn’t hunt, think, or become the "king of all creatures". However, this great evolutionary achievement comes at a significant cost. Let me share three facts about hypertension— First: Hypertension is the single largest cause of global mortality—no exceptions. Hypertension leads to cardiovascular and cerebrovascular diseases, causing 10.4 million deaths worldwide annually, far exceeding cancer-related deaths. Second: Globally, 22% of men aged 30 have hypertension—one in five. By age 75, regardless of gender, the prevalence reaches 90%. In China, there are about 300 million people with hypertension. Third: 49.4% of Chinese hypertension patients are unaware of their condition. Nearly half of hypertensive patients do not know they need treatment. Is this because people don’t get health checkups? No. More surprisingly, among those who do get checkups—those with health awareness—66.2% are unaware of their hypertension, nearly two-thirds. Of all hypertensive patients, only 16.8% have their blood pressure controlled to target levels. Are these facts shocking? Numbers are distant; we need to understand the reasons behind them. Why does hypertension cause so many deaths, yet most people underestimate its danger? The answer is: invisibility. Most hypertension cases are asymptomatic, so many patients are unaware. As the saying goes, "Great treachery appears loyal, great falsehood appears true." Transient blood pressure elevation is normal, but persistent elevation—hypertension—constantly harms our bodies. In about ten years, it evolves into cardiovascular and cerebrovascular diseases—coronary heart disease, myocardial infarction, stroke—leading to death. Why do most people not know they have hypertension, or even if they do, fail to act against it? Again, invisibility. As mentioned, hypertension disguises itself within normal physiological processes, causing almost no discomfort. This invisibility leads to two typical reactions: one, believing no symptoms means no disease, making it hard to detect; two, not seeing severe harm in the short term, so lacking motivation to treat. Both lead to high mortality. Why does everyone inevitably develop hypertension if they live long enough? Again, invisibility. On one hand, blood pressure elevation is invisibly embedded in human evolutionary mechanisms—unstoppable. On the other hand, hypertension doesn’t appear overnight. Before true hypertension develops, there’s a "pre-hypertension" stage. Seemingly normal blood pressure is just the tip of the iceberg. This window is the key period for reversing hypertension. Once missed, reversal becomes difficult. Many people fail to treat elevated blood pressure as a warning, missing this window and inevitably progressing to hypertension. Everything stems from invisibility. This trait makes hypertension dangerous, cunning, and seemingly unavoidable. Does this mean we must accept hypertension as fate? No. Although hypertension is an invisible demon, modern medicine can subdue it thoroughly. So, here’s what I most want to tell you—when it comes to hypertension, the greatest threat to your health is your cognition, not the level of modern medicine. Today, we can clearly detect, treat, and control hypertension. Evidence-based lifestyle prescriptions—diet, exercise, stress reduction—can maximally subdue this demon and keep you safe. That’s not the problem. The problem is, you must understand hypertension from the perspective of life itself. If your understanding is superficial, you won’t trust medicine, resulting in inconsistency between knowledge and action, ultimately harming yourself and others.
